Current patients are welcome to call (843) 347- 8380 at any time to set up an appointment. Lake City Family Medicine is proud to offer same day appointments with each of our providers in order to accommodate our patient’s busy schedules and to still allow them to see the provider of their choice.
Release of Medical Information
In order to keep your medical information confidential, medical records will only be released once our office has received a signed authorization form. You can stop by the office and request a form to fill out and mail, fax, or drop the form off at the front desk.
